Description

Medial forearm anatomy is centered on the flexor carpi ulnaris, a superficial muscle running along the ulnar (medial) border from the medial epicondyle of the humerus and proximal ulna toward the ulnar side of the wrist. Its muscle belly occupies the posteromedial compartment, with the tendon narrowing distally and tracking toward its insertion on the pisiform, continuing via the pisohamate and pisometacarpal ligaments to the hook of hamate and base of the fifth metacarpal. Immediately lateral (radial) to flexor carpi ulnaris lies flexor digitorum superficialis, while the ulnar nerve and ulnar artery course deep to the muscle in the proximal forearm and emerge more distally near the wrist. Clinically, this medial perspective matters because flexor carpi ulnaris forms a key landmark for locating the ulnar neurovascular bundle, both in anatomy teaching and in procedural practice. Distal to the elbow, ulnar nerve irritation can occur between the humeral and ulnar heads (cubital tunnel region), and tenderness or pain with resisted wrist flexion and ulnar deviation often points toward flexor carpi ulnaris tendinopathy. At the wrist, the relationship to the pisiform and Guyon canal helps explain ulnar neuropathy patterns that spare the dorsal ulnar cutaneous branch.
