Description
Running along the dorsoulnar border of the male forearm, the extensor carpi ulnaris (ECU) muscle belly lies superficial beneath the skin and fascia, hugging the posterior aspect of the ulna as it descends from the lateral epicondyle region toward the wrist. From a medial perspective, the ulna forms the dominant bony landmark, with the radius positioned more lateral and partially obscured, while the ECU tendon becomes more discrete distally as it tracks toward the ulnar head and carpus. At the wrist, the tendon’s course toward the base of the fifth metacarpal is implied by its alignment with the ulnar side of the carpal and metacarpal complex. Bone alignment is clear. This angle matters because ulnar-sided wrist pain often localizes to the ECU tendon and its fibro-osseous tunnel in the sixth dorsal compartment, where tendinopathy and stenosing tenosynovitis can mimic triangular fibrocartilage complex (TFCC) pathology. Supination, ulnar deviation, and resisted wrist extension can provoke ECU subluxation if the subsheath is torn, and a medial view anchored on the ulna helps orient that relationship without the distraction of the radial extensors. The close apposition to the ulna also clarifies why tenderness at the distal ulna can reflect tendon disease rather than joint inflammation. Use this artwork in upper limb anatomy teaching to reinforce forearm compartment orientation and distal tendon trajectories, or in a hand and wrist surgery chapter when describing dorsal compartment anatomy and approaches to the ulnar head, TFCC, and ECU stabilization.
