Description
Running along the extreme medial border of the anterior forearm, the flexor carpi ulnaris forms a prominent superficial column from the medial epicondyle region to the ulnar side of the wrist. Proximally, its humeral head arises from the common flexor origin at the medial epicondyle, while the ulnar head tracks along the posterior border of the ulna, positioning the muscle belly medial to flexor digitorum superficialis and anterior to the ulna. Distally, the tendon narrows and courses superficial to the wrist to reach the pisiform, with continuity to the hook of hamate and base of the fifth metacarpal via the pisohamate and pisometacarpal ligaments. Medial, long, unmistakable. Clinically, this anterior perspective is the practical way to teach how flexor carpi ulnaris relates to the ulnar neurovascular bundle, because the ulnar nerve passes between the humeral and ulnar heads near the elbow, a common site of entrapment in cubital tunnel syndrome. Follow the tendon to the ulnar side of the carpus and you also have a surface landmark for the Guyon canal region, where ulnar nerve compression can occur with cycling or ganglion cysts. Surgeons use this anatomy when planning ulnar-sided wrist approaches and when orienting themselves around the pisiform as a palpable bony reference.
