Posterior Perspective of the Flexor Carpi Ulnaris

Arising from the medial epicondyle of the humerus and the olecranon and posterior border of the ulna, the flexor carpi ulnaris forms a muscular column along the ulnar (medial) margin of the proximal forearm, with its fibers narrowing into a tendon as it courses distally toward the wrist. From a posterior perspective you would expect to appreciate its relationship to the subcutaneous border of the ulna, with the radius positioned more lateral and slightly anterior in anatomical position. Distally, the flexor carpi ulnaris tendon passes toward the pisiform on the ulnar side of the carpus, continuing via the pisohamate and pisometacarpal ligaments toward the hook of hamate and base of the fifth metacarpal. Clear bony landmarks anchor the orientation. This posterior angle matters because the flexor carpi ulnaris is the principal surface landmark overlying the ulnar nerve as it enters the forearm between the humeral and ulnar heads, a frequent entrapment site in cubital tunnel syndrome. Small changes in elbow flexion alter the relationship between the nerve, the FCU aponeurosis, and the medial epicondyle. Surgeons also use the FCU interval during medial elbow exposure and when planning approaches to the ulnar artery and nerve in the proximal forearm. A dependable landmark.
