Description
Running along the ulnar (medial) margin of the forearm, the flexor carpi ulnaris is highlighted from its proximal origin near the medial epicondyle and olecranon region toward its distal tendon at the ulnar side of the wrist. From a posterior perspective you can still appreciate its long fusiform belly bordering the superficial planes adjacent to the ulna, with extensor-side structures and the dorsal contours of the carpus providing orientation. Distally, the tendon courses toward the pisiform and the ulnar aspect of the hand, lying medial to the metacarpals and phalanges that are rendered beneath the overlying soft tissues. Bone landmarks guide the eye. Seeing flexor carpi ulnaris from behind matters because the ulnar border is a frequent landmark during posterior and posteromedial approaches to the elbow and forearm, and it helps clarify what sits just anterior to the palpable subcutaneous ulna. Clinically, the muscle’s two-headed origin is the gateway for the ulnar nerve as it enters the forearm between the humeral and ulnar heads, a common site of irritation in cubital tunnel syndrome and during medial elbow surgery. Distal tendon anatomy also frames pisiform-based pain patterns and the mechanics of ulnar deviation and wrist flexion, where flexor carpi ulnaris often acts in concert with flexor carpi radialis.
