Description
Running along the ulnar (medial) border of the posterior forearm, the flexor carpi ulnaris (FCU) is presented beneath the skin and superficial fascia in an adult male, its fleshy belly occupying the posteromedial contour from the medial epicondyle and proximal ulna toward the wrist. Distally, the muscle narrows into a prominent tendon that tracks superficial to the ulnar side of the carpus to its insertions at the pisiform, with continuations through the pisohamate and pisometacarpal ligaments toward the hook of hamate and base of the fifth metacarpal. Lateral to this border, the extensor mass of the posterior compartment creates the dorsal forearm volume, while the FCU remains the most medial landmark you can follow from elbow to wrist. Clear boundaries matter. This posterior skin-on relationship is where anatomy meets examination and decompression. The ulnar nerve passes posterior to the medial epicondyle, then enters the forearm between the humeral and ulnar heads of FCU, a frequent site of traction or compression in cubital tunnel syndrome and an anatomic detail that guides both provocative testing and surgical release. At the wrist, the FCU tendon helps you locate the ulnar neurovascular bundle as it approaches Guyon’s canal, and it provides a practical landmark when differentiating ulnar-sided wrist pain from extensor carpi ulnaris tendinopathy. Educators can drop this into upper-limb gross anatomy labs to teach surface anatomy and compartment logic, or into kinesiology modules when pairing FCU with wrist flexion and ulnar deviation mechanics.
