Great Saphenous Vein, Medial View

Description

Running subcutaneously along the medial border of the male lower limb, the great saphenous vein courses anterior to the medial malleolus, ascends over the medial surface of the tibia, then continues proximal to the knee toward the medial thigh. At the knee level it lies superficial to the medial joint line, tracking near the region of the pes anserinus and the medial head of gastrocnemius before passing into the thigh. Deep landmarks such as the tibia and femur provide orientation, with major muscle masses of the anterior thigh and posterior calf rendered beneath the superficial venous channel. This is a superficial system. It reads clearly against the deep compartment anatomy. Clinically, the medial knee segment is a frequent site of varicosities and tenderness in superficial thrombophlebitis because the vein is long, superficial, and exposed to hydrostatic pressure. The same corridor is where you plan and teach long saphenous vein harvesting for coronary artery bypass grafting, taking care to avoid injury to the saphenous nerve, which travels with the vein along the medial leg and can leave persistent medial calf or ankle dysesthesia. For venous access and ultrasound mapping, appreciating the vein’s relationship to the medial femoral condyle and tibial crest helps distinguish it from deeper venae comitantes of the posterior tibial system.
