Description
Running superficially along the posteromedial leg, the great saphenous vein is traced from the medial ankle, ascending behind the medial malleolus and along the medial border of the tibia toward the knee. At the popliteal level it passes posterior to the medial femoral condyle, sitting superficial to the gastrocnemius heads and plantaris tendon, while deeper structures of the popliteal fossa remain implied rather than emphasized. Proximally, the vein continues along the medial thigh, lying in the superficial fascia over the adductor canal region and approaching the saphenous opening. Calf and thigh contours are defined by the hamstrings superiorly and the gastrocnemius and soleus complex inferiorly, converging into the calcaneal (Achilles) tendon toward the heel. A posterior tracing is a practical way to teach where the great saphenous vein can be mistaken for, or distinguished from, the small saphenous vein, which typically ascends in the midline of the posterior calf toward the popliteal vein. Relation to the medial malleolus and medial femoral condyle matters during varicose vein mapping and during endovenous thermal ablation, when the saphenous nerve (closely associated with the vein below the knee) is at risk for neurapraxia and postoperative dysesthesia. Surface anatomy drives the procedure.
