Description
Medial exposure of the right knee centers on the popliteus, a flat triangular muscle lying deep on the posterior tibia within the floor of the popliteal fossa. Its tendon courses proximolaterally to the lateral femoral condyle, passing deep to the fibrous capsule and adjacent to the lateral meniscus, while the muscular belly fans inferomedially to insert on the posterior tibia superior to the soleal line. Superficial layers include the medial head of gastrocnemius and the semimembranosus and semitendinosus tendons forming the medial boundary of the fossa, with the femur, patella, tibia, and fibula providing bony context for the knee joint and proximal leg. Popliteus anatomy matters because it initiates knee flexion by internally rotating the tibia (or externally rotating the femur in weight bearing), the classic “unlocking” action that is easy to teach but hard to visualize without a deep posterior view. Injury patterns often involve the posterolateral corner, and distinguishing popliteus tendon pathology from lateral meniscal tears or lateral collateral ligament sprain affects both MRI interpretation and operative planning. During knee arthroscopy and posterior capsular work, its close relationship to the capsule and meniscus, and its depth relative to the popliteal neurovascular bundle, frames where dissection can go wrong. Small muscle, big consequences.
