Description
Plantar intrinsic musculature is presented in layered fashion across the sole (planta pedis), with the calcaneus posteriorly giving rise to the superficial flexor digitorum brevis centrally and the abductor hallucis and abductor digiti minimi forming medial and lateral borders. Deep to those lie the quadratus plantae inserting onto the tendons of flexor digitorum longus, plus the lumbricals tracking distally along the medial sides of the lateral four toes toward the extensor expansions. Blue tendon slips from flexor hallucis longus and flexor digitorum longus pass anteriorly toward the distal phalanges, crossing the midfoot over the tarsus and metatarsals and tying the muscular layers to the forefoot. Functionally, this plantar arrangement explains how the foot generates flexion at the metatarsophalangeal and interphalangeal joints while simultaneously stabilizing the longitudinal arches during gait, since the long flexor tendons are redirected and tensioned by quadratus plantae and the lumbricals. Compression or scarring along the medial plantar compartment is a common pain generator in tarsal tunnel syndrome, where the tibial nerve and its branches course posterior to the medial malleolus and then deep to abductor hallucis. Small structures, big consequences. The view also helps clarify why weakness or denervation of the intrinsic muscles in diabetic neuropathy promotes claw toe deformity and increases plantar forefoot pressure, predisposing to metatarsal head ulceration.
