Description
Running along the medial aspect of the male lower leg, the tibia descends to the medial malleolus, where the ankle transitions into the medial foot and the plantar surface comes into profile beneath the skin. Posterior to the tibial shaft, the deep posterior compartment tendons, tibialis posterior, flexor digitorum longus, and flexor hallucis longus, track distally and course behind the medial malleolus toward the tarsals, metatarsals, and phalanges. Within the medial plantar compartment, abductor hallucis forms the most superficial muscular mass along the medial border, with flexor hallucis brevis and the medial slip of flexor digitorum brevis and associated tendinous slips situated more laterally and deep, supporting the longitudinal arch. Medial plantar anatomy matters because it concentrates the structures responsible for dynamic arch control and the neurovascular bundle that is most often symptomatic in tunnel syndromes. Compression within the tarsal tunnel beneath the flexor retinaculum can irritate the tibial nerve and its medial and lateral plantar branches, producing plantar paresthesia that clinicians localize by tracing these relationships to the sustentaculum tali and the abductor hallucis fascia. Tendinopathy or tenosynovitis of tibialis posterior, just posterior and inferior to the medial malleolus, is a common driver of adult acquired flatfoot and is assessed and treated with this exact anatomy in mind.
