Description
Seen from a posterior perspective, the paired cerebral hemispheres terminate inferiorly in the occipital poles, their cortical gyri and sulci framing the midline cleft of the longitudinal fissure. Deep to the posterior cerebrum, the centrally placed thalamus sits on either side of the third ventricle, partially masked by surrounding white matter yet clearly positioned superior to the midbrain. Inferior and posterior to the cerebral hemispheres, the cerebellar hemispheres and vermis form a compact mass over the dorsal brainstem, with the medulla continuing caudally toward the spinal cord. Midline symmetry is emphasized. This angle is the one clinicians mentally reconstruct when localizing lesions to the posterior fossa and occipital region. Occipital pole involvement correlates with homonymous visual field defects, while crowding at the level of the cerebellum and brainstem underlies life threatening posterior fossa mass effect, where tonsillar herniation can compress the medulla and compromise respiration. The thalamus, although not a posterior surface structure, gains teaching value here because its deep position relative to the hemispheres and brainstem is hard to appreciate without a symmetric cut. Neuroanatomy faculty often use this posterior orientation to teach separation of supratentorial occipital cortex from infratentorial cerebellum, then bridge into thalamic relay nuclei and brainstem level organization in the same session. It also suits neurology texts and radiology teaching files when paired with axial or coronal MRI to reinforce three dimensional localization of thalamic hemorrhage, brainstem infarct, and posterior circulation territory patterns.
