Description
Viewed from the inferior (basal) surface, the cerebrum’s orbital gyri of the frontal lobes lie anteriorly, separated medially by the interhemispheric fissure, with the olfactory bulbs and tracts (CN I) running anteroposterior along the olfactory sulci. Just posterior to this region, the optic nerves (CN II) converge toward the optic chiasm above the sellar area, and the optic tracts sweep posterolaterally toward the midbrain. The midbrain, pons, and medulla oblongata occupy the midline posteriorly, with the cerebellar hemispheres and vermis positioned dorsally and inferiorly relative to the brainstem, and the cranial nerves emerging in ordered sequence from the ventral brainstem and pontomedullary junction. Nerves stand out. This perspective matters because it is the working map for correlating cranial nerve deficits with lesions at the skull base and ventral brainstem. Compression at the optic chiasm from a pituitary macroadenoma classically produces bitemporal hemianopia, while posterior circulation aneurysms and uncal herniation patterns are often taught using the relationship of CN III to the midbrain and the tentorial edge. It also supports surgical orientation in subfrontal and endonasal approaches where the olfactory tracts, optic apparatus, and perforating vessels define safe corridors. Use this asset in neuroanatomy and head and neck courses to teach basal brain landmarks, cranial nerve exits, and clinicopathologic localization, and in neurosurgical or neuroradiology publications discussing chiasmal syndromes, brainstem stroke patterns, or skull base approaches. It also fits patient-facing diagrams when explaining vision loss from sellar masses or multiple cranial neuropathies from meningeal disease.
