Presented from a posterior perspective, the deep femoral vein (vena profunda femoris, profunda femoris vein) courses proximally in the posterior and medial thigh within the adductor compartment, running in close company with the profunda femoris artery and receiving paired venous tributaries from perforating branches. Superiorly it approaches the femoral vein near the proximal thigh, deep to the adductor longus and adductor magnus and medial to the femur, while posterior muscular landmarks include gluteus maximus proximally and the hamstring group (biceps femoris, semitendinosus, semimembranosus) extending distally. Osseous orientation is provided by the posterior pelvis and proximal femur, including the region of the hip joint and greater trochanter. Deep and superficial planes are easy to separate here. This posterior rendering matters because the profunda system carries a large share of venous return from the deep thigh and becomes a key collateral pathway when the superficial femoral vein is thrombosed or chronically obstructed. Surgeons and interventionalists rely on these relationships when interpreting duplex ultrasound windows in the posterior and medial thigh, planning venous recanalization, or navigating around perforators during approaches to the adductor magnus and hamstring origin. Posterior thigh trauma can also injure perforating vessels, and knowing where the deep femoral vein sits relative to the femur and adductor magnus helps anticipate deep hematoma and compartment-related swelling.
