Description
Posterior to the femur, the femoral vein is traced proximally from the adductor hiatus into the popliteal vein and distally toward the deep venous channels of the leg, rendered in blue against the posterior thigh musculature. Running in the adductor canal, it sits anteromedial to the femur and deep to sartorius and the adductor longus and magnus, then transitions inferiorly to lie adjacent to the popliteal artery in the popliteal fossa. Superficial landmarks in this posterior view include gluteus maximus superiorly, the hamstrings (biceps femoris laterally, semitendinosus and semimembranosus medially) spanning the posterior thigh, and the gastrocnemius heads framing the knee region. Posterior orientation matters because it clarifies the femoral vein’s continuity with the popliteal system at the adductor hiatus, a common point of reference when teaching deep venous anatomy and collateral pathways. Deep vein thrombosis often begins in the calf veins and propagates proximally into the popliteal and femoral segments, and this depiction makes the anatomic course behind the knee and into the thigh easy to follow. It also supports procedural planning when discussing popliteal venous access, compression ultrasonography targets at the common femoral and popliteal levels, and the relationship of the vein to posterior thigh compartments where hematoma can track after trauma.
