Description
Seen from a posterior perspective, the cranial dura mater (pachymeninx) is presented in broad sheet form, spanning the inner surface of the occipital and posterior parietal regions and converging toward the midline along the course of the superior sagittal sinus. The tentorium cerebelli is visible as a firm dural fold projecting anteromedially from the posterior cranial fossa, forming a transverse partition between the occipital lobes superiorly and the cerebellum inferiorly. Laterally, the tentorial attachments track toward the petrous ridges, while the midline reflections continue toward the internal occipital region where dural folds intersect around the confluence of sinuses. Orientation matters. A posterior view of the dura highlights the tentorium cerebelli as the key barrier separating supratentorial and infratentorial compartments, a distinction that directly frames patterns of transtentorial (uncal) herniation and posterior fossa mass effect. The same geometry is used in operative planning for suboccipital and retrosigmoid approaches, where dural opening and venous sinus preservation dictate exposure and bleeding risk. Venous sinus relationships are the point.
