Emerging from the posterior cervical spine, the longissimus capitis is rendered bilaterally as a longitudinal column of muscle running superolaterally from the transverse processes of the mid to lower cervical vertebrae (often extending from upper thoracic levels) toward its insertion on the mastoid process of the temporal bone. Medial to it, semispinalis capitis occupies the deeper midline mass over the cervical laminae and spinous processes, while splenius capitis sits more superficially and laterally, forming an oblique sheet that also targets the mastoid region and lateral superior nuchal line. Portions of the occipital bone and posterior cranial sutures cap the field superiorly, with C1 to C7 providing bony landmarks from the atlas to the prominent spinous process of C7. Scapular outlines anchor the inferior-lateral margins. Posterior exposure of the upper cervical musculature matters when you need to explain how extension and ipsilateral rotation are generated across the atlanto-occipital and midcervical segments, because longissimus capitis and splenius capitis act as paired extensors but become rotators when contracting unilaterally against the deeper semispinalis mass.
