Description
Posterior neck and upper thorax anatomy is presented with the occipital bone superiorly and the cervical vertebrae aligned in the midline, transitioning inferiorly into the upper thoracic spine and ribs. Deep to the superficial trapezius, the longissimus capitis runs longitudinally along the posterolateral cervical region, positioned lateral to semispinalis capitis and medial to the more superficial splenius capitis. Its fibers course superiorly toward the mastoid process of the temporal bone, while its inferior attachments arise from the transverse processes of upper thoracic vertebrae and lower cervical vertebrae, placing it as a key extensor-rotator unit of the head and neck. Relationships to the cervical articular pillars and nuchal ligament are clear. A deep layer worth seeing. Longissimus capitis is a frequent pain generator in cervicogenic headache and postural neck strain, where myofascial trigger points can refer pain to the temporal region and behind the ear, mimicking otologic symptoms because of its mastoid insertion. The posterior approach relevance is practical: when trapezius and splenius are reflected in posterior cervical exposures, the longissimus and semispinalis planes guide safe dissection toward the lamina and facet joints while staying oriented around dorsal rami and the greater occipital nerve pathway through semispinalis capitis. That surgical layering is easy to lose in a flat diagram.
