Description
Posterior anatomy of the adult male is presented from the occiput to the calcaneus, with the vertebral column forming the midline axis beneath layered back musculature. Superiorly, trapezius spans from the nuchal region and thoracic spinous processes to the scapular spine and acromion, while deltoid caps the lateral shoulder and frames the posterior axillary fold created by latissimus dorsi. Paraspinally, the erector spinae columns track longitudinally on either side of the spinous processes, and the posterior arm is defined by triceps brachii running from the humerus and scapula to the olecranon. Inferiorly, gluteus maximus overlies the posterior pelvis, the hamstrings descend from the ischial tuberosity toward the knee, and gastrocnemius and soleus converge distally into the calcaneal (Achilles) tendon. This posterior perspective matters because it organizes the superficial and deep back into clinically meaningful planes: trapezius and latissimus dorsi as broad mobilizable sheets, and the erector spinae as the principal extensor mass adjacent to the spinous processes. It is the working map for posterior approaches to the spine, where the intermuscular interval between multifidus and longissimus is developed in Wiltse exposures to reduce midline muscle stripping and postoperative pain. It also supports examination patterns, such as observing scapular position and posterior shoulder contour in spinal accessory nerve palsy, or tracking the hamstrings and gastrocnemius when differentiating proximal hamstring avulsion from Achilles tendinopathy.
