Description
Posterior anatomy frames the spinal cord within the vertebral canal, extending inferiorly to the conus medullaris and giving rise to paired dorsal and ventral roots that unite as spinal nerves at each intervertebral foramen. Segmental dorsal rami course posteriorly into the paraspinal region while ventral rami sweep laterally to form the major plexuses, with the brachial plexus projecting into the shoulder and upper limb and the lumbosacral plexus continuing into the gluteal region and lower limb. Peripheral nerve trunks then track distally along the limbs, aligned with familiar surface corridors, from the posterior arm into the forearm and hand and from the posterior thigh toward the leg and foot. Midline orientation remains clear throughout. A posterior perspective is the clearest way to teach how spinal nerve roots and rami relate to the vertebral column and paraspinal musculature, an anatomic relationship that underpins dermatomes, myotomes, and deep tendon reflex testing. It also maps cleanly onto procedures performed from the back, including lumbar puncture between L3 to L4 or L4 to L5, neuraxial anesthesia, and posterior approaches to the spine where iatrogenic injury to dorsal rami and segmental vessels can drive postoperative pain syndromes. Clinically, the distribution shown supports localization in radiculopathy from cervical or lumbar disc herniation and in entrapment patterns along long peripheral nerves.
