Description
Arising along the lesser curvature of the stomach, the right gastric vein courses superiorly and medially toward the porta hepatis, where it joins the portal venous system. From a posterior perspective, the pyloric region and proximal duodenum sit inferior and to the right of the gastric body, placing the vein in close relationship to the pylorus and the hepatoduodenal ligament. Smaller venules from the antrum and pyloric canal converge into a more defined right gastric trunk as it approaches the portal vein region. Posterior orientation matters here because it mirrors the operative reality when the lesser omentum is elevated and the stomach is mobilized to expose the suprapancreatic vasculature. Bleeding from the right gastric vein is a common nuisance point during distal gastrectomy, pyloroplasty, and lymphadenectomy along the lesser curvature (stations 3 and 5), and this vessel is frequently ligated early to improve access and reduce venous back-bleeding. Portal hypertension also recruits and dilates gastric venous channels; understanding where the right gastric vein feeds the portal system helps explain patterns of congestive gastropathy and contributes to mapping portosystemic collateral flow around the gastroesophageal junction. In teaching settings, this plate fits well in a foregut block, abdominal vascular anatomy lab, or a surgical anatomy chapter focused on the lesser omentum and portal triad relationships.
