Description
Posteriorly, the paired thalami sit as ovoid masses of the diencephalon on either side of the midline, their caudal poles widening into the pulvinar where the medial and lateral geniculate bodies lie. Superior and lateral to the brainstem, each thalamus occupies the medial wall of the cerebral hemisphere, deep to the cerebral cortex and its sulci and gyri. Inferiorly the thalamic region approaches the midbrain, while the spinal cord continues caudally from the visible brainstem. Bilateral symmetry is clear. A posterior perspective makes the geniculate complexes easier to teach in context: the lateral geniculate body is the thalamic relay for the optic tract on its way to the primary visual cortex, and the medial geniculate body relays auditory pathways toward the temporal lobe. This is the view you want when explaining how posterior cerebral artery territory infarcts can involve the pulvinar and geniculate region, producing visual field deficits or less common auditory processing disturbances, and when orienting learners to why small deep lesions can look deceptively limited on imaging but cause specific sensory syndromes.
