Seen in sagittal section with a lateral perspective, the thalamus occupies the central diencephalon as an ovoid mass of deep gray matter, positioned inferior to the body of the corpus callosum and fornix, and superior to the hypothalamus. Anteriorly it approaches the interventricular foramen region and the anterior limb of the internal capsule, while posteriorly it tapers toward the pulvinar adjacent to the midbrain tectal level. Laterally, the thalamic surface relates to the internal capsule (posterior limb) and lentiform nucleus, and medially it borders the third ventricle. Major internal landmarks implied by this cut include the massa intermedia when present, the intralaminar region along the medullary laminae, and the proximity of the lateral and third ventricles. A lateral-sagittal presentation is the cleanest way to teach the thalamus as a relay rather than a single structure, because the slice keeps the hypothalamus, midbrain, and corpus callosum in the same plane while preserving the anterior to posterior nuclear gradient. That matters clinically: lacunar infarcts of the thalamogeniculate territory can produce pure sensory stroke, and involvement of the ventral posterolateral nucleus or adjacent posterior limb of the internal capsule helps explain combined sensory and motor deficits. Pain syndromes after thalamic injury (Dejerine-Roussy) also make more sense when you can track how sensory pathways converge before projecting to primary somatosensory cortex.
