Description
Anteriorly oriented anatomy of the male diencephalon places the paired thalami as ovoid gray-matter masses flanking the third ventricle, separated by the midline slit of the ventricular cavity and often bridged by the interthalamic adhesion. Superior and lateral to each thalamus, the internal capsule and corona radiata of the cerebral hemispheres frame the relay nuclei before their projections fan to the cortex. Inferiorly, the thalamus tapers toward the mesencephalon at the level of the midbrain, while the ventricular system is emphasized in blue, including the third ventricle and its continuity with the cerebral aqueduct and lateral ventricles. Cranial nerve roots (yellow) emerge from the brainstem below, providing orientation to the midbrain, pons, and medulla. Clinically, an anterior-facing depiction of the thalamus matters because vascular territories and surgical corridors are taught in relation to the third ventricle and internal capsule: paramedian thalamic infarcts from thalamoperforating arteries can produce decreased arousal, vertical gaze palsy, and memory disturbance, while lateral thalamic strokes may mimic internal capsule lesions with dense hemisensory loss. A common teaching trap is confusing thalamic symptoms with cortical deficits; seeing the thalamus positioned medial to the internal capsule clarifies why small deep lesions can create disproportionate sensory syndromes without aphasia or neglect.
