Description

From a posterior perspective, the paired thalami sit deep to the cerebral hemispheres, positioned superior to the midbrain and anterior to the cerebellum and dorsal brainstem. Each thalamus expands posteriorly into the pulvinar, forming the dominant rounded contour of the posterior thalamus and lying medial to the posterior limb of the internal capsule (not externally visible but implied by the deep location). Surrounding context includes the occipital poles of the cerebrum laterally and superiorly, with the cerebellar hemispheres and vermis inferiorly in the posterior fossa. Midline symmetry is clear. Pulvinar-focused views matter because the posterior thalamus is a common teaching stumbling block when learners try to map cortical visual and attentional networks back to diencephalic relay nuclei. The pulvinar participates in visual attention and integration through connections with the occipital, parietal, and temporal association cortices, and it sits close to pathways implicated in posterior circulation strokes. In neuroimaging correlation, lesions in the posterior thalamus can produce contralateral sensory loss with associated visuospatial neglect or higher-order visual processing deficits, patterns that help separate thalamic injury from primary occipital cortex pathology.
