Description
Running along the deep surface of latissimus dorsi, the thoracodorsal vein is visualized in a posterior view as it courses superolaterally toward the posterior axillary fold. Surrounding landmarks include trapezius superiorly, rhomboid major and minor adjacent to the medial border of the scapula, and teres major forming the inferior margin of the axilla, with the posterior deltoid lying lateral over the glenohumeral region. The vessel’s trajectory is shown in relation to the scapula and the thoracic wall, with muscle fiber direction and tendinous insertions providing orientation. Posterior exposure clarifies how the thoracodorsal vein travels with the thoracodorsal artery and nerve (the thoracodorsal neurovascular bundle) along the undersurface of latissimus dorsi before draining into the axillary venous system. This corridor matters in latissimus dorsi flap surgery, including pedicled and free transfers for breast and chest wall reconstruction, where pedicle length, kinking at the axillary fold, and inadvertent injury during axillary dissection can compromise venous outflow.
