Seen from the posterior aspect, the arterial tree of the male upper limb runs distally from the axillary artery into the brachial artery along the posterior compartment of the arm, approaching the cubital fossa at the level of the elbow. Around the distal humerus, the profunda brachii (deep artery of the arm) and its radial collateral and middle collateral branches track with the radial nerve in the spiral (radial) groove, then contribute to the periarticular anastomoses. Distal to the elbow, the brachial artery divides into the radial artery laterally and the ulnar artery medially, continuing down the posterior forearm toward the wrist with smaller arteriolar branches forming collateral channels. A superficial blue vessel along the lateral margin is consistent with the cephalic vein. Posterior arm vasculature matters because the deep brachial system defines the corridor for posterior approaches to the humeral shaft and explains the bleeding pattern in spiral groove injuries. Radial nerve palsy after midshaft humeral fracture often travels with arterial injury to the profunda brachii or its collaterals, and this view makes that anatomic pairing hard to ignore. Elbow perfusion is another teaching point, since the collateral networks around the lateral epicondyle and olecranon maintain distal flow when the brachial artery is compressed, clamped, or traumatically occluded.
