Description

Running between adjacent lumbar laminae, the ligamenta flava form paired yellow, elastic bands that line the posterior wall of the vertebral canal. From a posterior perspective, the spinous processes sit in the midline, with the laminae flaring laterally toward the facet (zygapophysial) joints and transverse processes, while the ligamentum flavum bridges each interlaminar space deep to the bony arch. Intervertebral discs lie anterior to this plane between vertebral bodies, but their segmental levels help orient the viewer to L1 through L5. Clinically, the ligamentum flavum matters because it is the structure the surgeon or proceduralist encounters immediately deep to the lamina during lumbar laminectomy, laminotomy, or interlaminar epidural access. Hypertrophy, fibrosis, or infolding of this elastic ligament contributes to central canal stenosis, often at L4 to L5, and it can narrow the lateral recess where the traversing nerve root courses.
