Description
Running along the posterior wall of the lumbar vertebral canal, the ligamenta flava (yellow ligaments) span between adjacent laminae from the inferior margin of the lamina above to the superior margin of the lamina below. In lateral profile, the elastic band sits posterior to the dural sac and posterior to the intervertebral disc, while remaining anterior to the spinous processes and the interspinous region. Pedicles and vertebral bodies define the anterior boundary of this segment, with the intervertebral foramina opening laterally where the lumbar spinal nerves would exit in close relationship to the facet (zygapophysial) joints. Elastic recoil is the point. The lumbar ligamentum flavum resists flexion and helps the laminae return toward extension, but its thickness also makes it a frequent contributor to central canal or lateral recess stenosis when hypertrophied, folded, or calcified. That pathology matters in routine lumbar decompression: surgeons often resect the ligamentum flavum during laminectomy or laminotomy to expose the epidural space and relieve compression of the traversing nerve roots, and the lateral extent of resection near the medial facet influences postoperative stability.
