Description
Oriented from a posterior perspective, the colon is arranged in its characteristic frame, with the ascending colon on the viewer’s right, the transverse colon superiorly, and the descending and sigmoid segments sweeping inferiorly toward the pelvis. Blue venous channels track along the mesenteric border, and the ileocolic vein is identified draining the terminal ileum and cecal region before coursing proximally within the mesentery toward the superior mesenteric vein. Smaller ileal and colic tributaries converge medially, running deep to the bowel loops and within the mesenteric connective tissue, while epiploic appendages appear as yellow fat tags along the colonic surface. Relationships are presented in surgical terms: the ileocolic pedicle lies inferior to the right colic drainage and lateral to the mesenteric root. Attention to the ileocolic vein matters most at the right lower quadrant. During ileocecal resection or right hemicolectomy, controlled division of the ileocolic vessels at their origin reduces blood loss and defines the central vascular ligation plane, but it also places the superior mesenteric vein and adjacent lymphatics at risk if the posterior mesenteric plane is entered too medially. Portosystemic collateral pathways can also enlarge with portal hypertension, and venous caliber and confluence patterns help explain unexpected variceal bleeding around the cecum or terminal ileum. This is a dependable roadmap. Use this illustration in anatomy and surgical anatomy curricula when teaching midgut venous drainage, mesenteric planes, and vascular pedicles, or in operative atlases and CME material covering right colectomy, ileocecal resection, and D3 lymphadenectomy concepts.
