Description

Centered on the right lower quadrant, the ileocolic artery is traced from its origin at the superior mesenteric artery as it courses inferiorly and to the right within the mesentery toward the ileocecal junction. Anteriorly, its ileal branches run along the terminal ileum while colic branches ascend on the medial aspect of the ascending colon. Near the cecum, the anterior and posterior cecal arteries and the appendicular artery are typically represented, with the latter descending toward the vermiform appendix from the ileocolic arcade. Mesenteric fat frames the vessels, and the haustra, taeniae coli, and appendices epiploicae provide surface landmarks on the colon. Appreciating this arterial pattern matters when you are thinking through right-sided colonic perfusion and the surgical planes used in ileocecal resection and right hemicolectomy. The ileocolic pedicle is routinely ligated during oncologic lymphadenectomy for cecal and ascending colon cancers, and variable branching can influence the risk of ischemia at the ileocolic anastomosis. Acute appendicitis also lives in this territory, where compromise of the appendicular artery can contribute to gangrene, and where endovascular embolization or inflammatory thrombosis may alter expected enhancement patterns on contrast CT angiography.
