Seen from the posterior trunk beneath intact skin, the internal oblique abdominis forms a thick muscular sheet rising from the iliac crest and thoracolumbar fascia, then sweeping superomedially toward the lower ribs and linea alba. Its posterior border sits lateral to the lumbar vertebral column and the paraspinal mass (erector spinae), and it lies anterolateral to quadratus lumborum in the flank. Superficially, latissimus dorsi crosses the lower thorax and upper lumbar region, while the internal oblique occupies the deeper lateral abdominal wall just inferior to the costal margin and superior to the iliac crest. Thick posterior fibers stand out. That posterior thickness matters in real anatomy, because it helps define the boundaries of the inferior lumbar triangle (Petit), where the posterior border of external oblique forms the anterior boundary and latissimus dorsi forms the posterior boundary, a setup that explains the typical weak points for lumbar hernia. The same flank relationship guides posterior and lateral abdominal wall approaches, including retroperitoneal access and the trajectory of a transversus abdominis plane (TAP) block where anesthetic spreads between internal oblique and transversus abdominis.
