Description
Posteriorly, the descending branch of the lateral circumflex femoral artery runs inferiorly along the lateral thigh, typically emerging from the profunda femoris system near the intertrochanteric region and tracking deep to the tensor fasciae latae and rectus femoris toward the distal femur. Proximally it lies lateral to the femoral shaft, then approaches the lateral intermuscular septum as it descends, with perforating connections toward the posterior compartment. Superficial venous channels and deeper paired venae comitantes may be seen paralleling the arterial course, set against the gluteal region and posterior thigh contours. Long axis. Clear trajectory. Surgeons care about this vessel because it supplies the anterolateral thigh and contributes to the periarticular anastomoses around the hip and lateral knee, territories that matter when planning incisions, flap harvest, and fracture fixation. During an anterolateral thigh (ALT) flap, perforators often arise from this descending branch, and their relationship to the septum between rectus femoris and vastus lateralis helps predict pedicle length and dissection planes. Orthopedic approaches around the greater trochanter also risk injuring branches of the lateral circumflex femoral system, which can complicate hemostasis in muscular compartments. Use this posterior perspective for teaching lower limb vascular anatomy in gross anatomy and surgical anatomy courses, where students need to reconcile the profunda femoris branches with compartmental boundaries in the thigh.
