Arising from the profunda femoris artery just inferior to the inguinal ligament, the lateral femoral circumflex artery courses laterally and anteriorly across the proximal thigh before dividing into its ascending and descending branches. The ascending branch tracks superiorly toward the anterior superior iliac spine and along the intertrochanteric region, while the descending branch runs distally deep to the sartorius and rectus femoris, paralleling the femoral shaft toward the knee. Medial to this branching point, the femoral artery continues down the femoral triangle and adductor canal, with the great saphenous vein visible superficially along the anteromedial thigh. Arterial and venous pathways are separated cleanly by depth. Rapid division of the lateral circumflex femoral system matters because it defines a frequent source of bleeding and a dependable conduit for reconstruction. During anterolateral thigh (ALT) flap harvest, perforators typically originate from the descending branch, and their relationship to the vastus lateralis and rectus femoris dictates dissection planes and pedicle length. Orthopaedic exposures around the hip and proximal femur, including direct anterior approaches and fixation for intertrochanteric fractures, can place the ascending branch at risk, with hematoma or pseudoaneurysm as recognizable complications.
