Description
Running obliquely from the transverse processes of the thoracic vertebrae to the ribs just inferiorly, the levatores costarum appear as short, paired slips along the posterolateral thoracic wall. Their fibers course inferolaterally from the vertebral column toward the rib angles, sitting deep to the erector spinae group and blending functionally with the external intercostal layer between adjacent ribs. The thoracic vertebrae, posterior rib cage, and pelvis form the bony framework, with the levatores costarum aligned segmentally from the upper to mid thoracic levels. Clinically, this posterior perspective helps clarify why these muscles are easy to overlook in routine teaching yet relevant when interpreting posterior thoracic pain and segmental dysfunction near the costovertebral and costotransverse joints. Because the levatores costarum can assist rib elevation and stabilize the rib-vertebra articulation, irritation from thoracic facet arthropathy, rib dysfunction, or paraspinal strain may refer pain along the posterior rib angles and mimic intercostal neuralgia.
