Description
Posterior thoracic musculature is presented with the levatores costarum emerging from the transverse processes of the lower cervical and upper thoracic vertebrae and descending inferolaterally to the ribs. Short (breves) slips insert on the rib immediately below, while long (longi) slips span to the second rib below, lying deep to the erector spinae group and superficial to the costotransverse joints. Medially, their fibers hug the spinous and transverse process region, and laterally they blend into the outer surface of the rib near the angle, just lateral to the vertebral column. Small muscles. Clear attachments. Functionally, this posterior view is the one you want when teaching how segmental paraspinal muscles couple rib motion to thoracic extension and ipsilateral rotation, because the oblique course of each levator bridges vertebra to rib at the costovertebral complex. Clinically, these slips matter in upper thoracic and cervicothoracic pain patterns, where overuse, trigger points, or strain can mimic costovertebral joint dysfunction, and they become relevant landmarks during posterior approaches that traverse the paraspinal gutter toward the ribs. The relationship to the erector spinae also helps explain why these small elevators are often overlooked on surface anatomy yet encountered in deep dissection.
