Description
Running obliquely from the transverse processes of the mid to lower thoracic vertebrae to the ribs immediately inferior, the levatores costarum appear as paired slips deep to the dermal layer along the posterior thoracic wall. Each fascicle courses inferolaterally from the vertebral column toward the rib angle, lying lateral to the spinous processes and medial to the more superficial erector spinae mass. Short segments (levatores costarum breves) span to the next rib, while longer slips (longi) extend across an intercostal space to the second rib below. Small muscles. Clear attachments. Functionally, these fibers sit at the intersection of rib mechanics and segmental thoracic stability, contributing to rib elevation during inspiration and blending with the external intercostal layer in the posterior intercostal spaces. Their close relationship to the costotransverse joints and the dorsal rami makes them relevant when teaching posterior thoracic pain patterns and when correlating with targeted injections or radiofrequency procedures near the costotransverse region.
