Description
Arising as paired slips from the transverse processes of C7 and T1 to T11, the levatores costarum descend inferolaterally to insert on the rib immediately below (and sometimes the second rib below) near the angle of the rib. From this posterior perspective, each muscle spans the costotransverse region, lying deep to the iliocostalis and longissimus portions of the erector spinae and superficial to the posterior intercostal spaces. Thoracic vertebral spinous processes form the midline reference, while the ribs sweep laterally around the thorax; the scapulae and proximal humeri frame the upper field but remain secondary. Fine, segmental anatomy. Functionally, these slips act as accessory elevators of the ribs and blend mechanically with the posterior intercostal musculature, so the view is useful when teaching how thoracic cage expansion couples with thoracic spine motion during inspiration. Their segmental innervation from dorsal rami means they are encountered in posterior paraspinal dissections and may be implicated in focal thoracic back pain near the rib angles, where costovertebral and costotransverse joint irritation can coexist. For surgeons and pain clinicians, recognizing the levatores costarum helps orient dissection planes in posterior thoracotomy approaches and clarifies why tenderness can localize lateral to the spinous processes rather than at the midline.
