Seen from a posterior perspective, the levatores costarum appear as paired, segmented muscle slips spanning from the transverse processes of the lower cervical and upper thoracic vertebrae to the ribs just lateral to their tubercles. Each fascicle runs inferolaterally, crossing one intercostal space (levatores costarum breves) or two spaces (levatores costarum longi), with attachments typically extending from C7 and T1 through about T11. Deep to the superficial back muscles and lying close to the costovertebral joints, these thin elevators track alongside the posterior elements of the thoracic cage while the vertebral column, ribs, and scapulae provide bony orientation.
