Description
Positioned on the posterior thoracic wall, the levatores costarum appear as paired, slender muscle slips arising from the transverse processes of C7 through T11 and running inferolaterally to the rib immediately below, inserting between the rib tubercle and the costal angle. From a posterior view of the male upper back, these fibers sit deep to the scapulae and superficial to the rib shafts, flanking the midline thoracic spinous processes and costotransverse joints. Their segmental arrangement parallels the ribs as they sweep laterally from the vertebral column. Small muscles. Clear landmarks. Functionally, the levatores costarum link vertebral mechanics to rib kinematics, and this posterior perspective makes their relationship to the costotransverse articulations and the dorsal rami of thoracic spinal nerves easier to teach than in an anterior thoracic cage plate. During deep inspiration, their line of pull favors rib elevation (most evident in the upper thorax), while the longer levatores costarum longi span two rib levels and can be confused with small bundles of the iliocostalis or intercostal musculature on less specific diagrams. Clinically, these slips matter when interpreting posterior thoracic pain patterns and planning posterior approaches that work near the transverse processes, including costotransversectomy for thoracic disc herniation or exposure for instrumentation, where small segmental muscles are detached to reach the rib head and pedicle corridor.
