Description
Running obliquely from the transverse processes of the lower cervical and upper thoracic vertebrae toward the posterior rib angles, the levatores costarum appear as a series of short, segmented slips on the deep aspect of the posterior thoracic wall. Each muscle belly courses inferolaterally from the vertebral column to insert on the rib immediately inferior (levatores costarum breves) or to span two ribs (levatores costarum longi), lying medial to the scapulae and deep to the rhomboid major and the middle fibers of trapezius. Posteriorly, the ribs arc laterally from the costovertebral and costotransverse joints, framing the muscular slips as they cross the intercostal spaces near the rib tubercles. Bony landmarks include the thoracic spinous processes in the midline and the medial borders of the scapulae laterally. Segmental anatomy matters here. The levatores costarum are small, but their attachments place them next to the costotransverse joints, where mechanical irritation and thoracic zygapophyseal dysfunction can refer pain around the hemithorax and mimic intercostal neuralgia. Their line of pull assists rib elevation during inspiration, and in scoliosis or postural kyphosis these slips can become asymmetric, which helps explain focal tenderness near the rib angles and altered thoracic excursion. This posterior relationship also provides orientation for approaches that work near the rib heads, including thoracic paravertebral blocks and targeted injections at the costotransverse region. Educators can drop this view into gross anatomy or kinesiology modules to clarify how deep thoracic muscles relate to the scapulothoracic musculature and the vertebra-rib articulations.
