Description

Running bilaterally along the posterior abdominal wall, the lumbar veins are shown as paired, segmental vessels coursing adjacent to the lumbar vertebrae and the medial margins of psoas major before converging medially toward the inferior vena cava. Superiorly, they relate to the inferior border of the rib cage and the posterior intercostal region, while inferiorly they approach the iliac crests and the pelvic brim. Short transverse channels link each lumbar vein to the vertebral venous plexuses, and the ascending lumbar vein is appreciable as a longitudinal conduit on each side bridging the segmental tributaries. Bony landmarks include the lumbar spinous processes in the midline and the iliac bones laterally. Posterior lumbar venous anatomy matters because these vessels provide a clinically relevant collateral pathway between the common iliac system and the azygos or hemiazygos systems via the ascending lumbar veins, a route that can enlarge in inferior vena cava obstruction. Surgeons encounter them during posterior and lateral approaches to the lumbar spine, retroperitoneal exposure, and nephrectomy, where lumbar vein avulsion near the cava or renal vein can produce brisk hemorrhage. Use this posterior perspective in gross anatomy and radiologic anatomy teaching to orient learners to segmental venous drainage in the lumbar region and to pair with axial CT or MR images where lumbar veins appear as thin, variable-caliber structures near psoas.
