Description

Posteriorly, the medial meniscus appears as a C shaped fibrocartilaginous wedge interposed between the medial femoral condyle superiorly and the medial tibial plateau inferiorly, with the posterior horn occupying the posteromedial corner of the tibiofemoral joint space. Its peripheral border blends with the joint capsule along the medial margin, while the free inner edge faces the intercondylar region. The posterior aspect of the tibial plateau and adjacent articular cartilage are exposed, and a cruciate ligament band is plausibly seen centrally, coursing from tibia to femur deep within the intercondylar notch. Patella and trochlear surfaces fall outside the posterior field. A posterior view is where the posterior horn matters most. Tears here, classically longitudinal or complex at the posterior horn or its root attachment, can destabilize hoop stresses and mimic early medial compartment osteoarthritis on exam and imaging, with joint line tenderness and mechanical catching. For arthroscopy, the posteromedial compartment is also where the surgeon must understand the relationship of the posterior horn to the capsule and nearby neurovascular structures, since portal placement and meniscal repair sutures traverse the posterior capsular region. Small landmark, big consequences.
