Description
Arising from the posterior division of the mandibular nerve (V3), the meningeal branch (nervus spinosus) courses posteriorly and superiorly to re-enter the cranial cavity through the foramen spinosum. From a posterior perspective, the relationship between V3 in the infratemporal fossa and the middle cranial fossa is emphasized, with the branch tracking medially toward the dura mater adjacent to the sphenoid spine. The nearby middle meningeal artery commonly accompanies it through the same foramen. Small dural twigs spread over the floor of the middle cranial fossa. Clear topology. Clinically, this is the key neural companion of the middle meningeal artery in extradural space, and its proximity explains why middle meningeal artery injury and epidural hematoma can present with meningeal pain transmitted via V3. The view also supports surgical orientation during approaches to the foramen ovale and foramen spinosum, including percutaneous procedures for trigeminal neuralgia and skull base exposure where the foramen spinosum is used as a landmark to identify the middle meningeal artery before dural elevation.
