Description
Lateral cranial anatomy is rendered with the cerebral hemisphere in profile, its gyri and sulci overlying the middle cranial fossa and the dural compartment. A yellow recurrent meningeal branch of the mandibular nerve (nervus spinosus) is traced proximally from V3 at the foramen ovale region and then reflected posteriorly to re-enter the cranial cavity through the foramen spinosum in company with the middle meningeal artery (red) and accompanying veins (blue). From that entry point the meningeal ramus fans across the dura mater of the middle cranial fossa, lying deep to the squamous temporal bone and lateral to the sella region. It is a small nerve with an outsized teaching value. Clinically, this is the sensory nerve classically implicated in pain referral from dural irritation in the middle fossa, including headache patterns associated with meningeal inflammation or traction on the dura during neurosurgical exposure. Its close relationship to the middle meningeal artery matters when you discuss epidural hematoma after temporal bone fracture, because the vascular injury occurs in the same dural plane the nervus spinosus supplies. Surgeons working around the infratemporal fossa, foramen ovale, or foramen spinosum also use these foramina as hard bony landmarks to orient the mandibular division of the trigeminal nerve and adjacent ganglionic structures. Neuroanatomy and head and neck courses can use this plate to teach the recurrent course of the meningeal branch of V3, its dural distribution, and its relationship to meningeal vessels in a single lateral composition. It also fits well in atlases, operative anatomy chapters, and patient education materials discussing dural pain pathways and epidural bleeding.
