Arising from the mandibular nerve (V3) in the infratemporal region, the meningeal branch (nervus spinosus) turns posteriorly and superiorly to re-enter the middle cranial fossa through the foramen spinosum. Closely accompanying the middle meningeal artery, it courses medial to the mandibular division and deep to the lateral pterygoid region, then spreads along the dura mater after crossing the cranial base. Yellow neural pathways are set against red arteries and blue venous channels, with the cerebrum and brainstem superiorly and the cervical spinal cord continuing inferiorly. This relationship to the foramen spinosum and middle meningeal vessels is the teaching point. During epidural hematoma from middle meningeal artery laceration, dural innervation carried by V3 meningeal fibers contributes to the character and distribution of acute headache and temporal pain. The anatomy also matters in surgical and interventional corridors, including subtemporal and pterional approaches, middle cranial fossa exposure for skull base work, and endovascular or open procedures involving the middle meningeal artery, where adjacent neural structures can be irritated.
