Description
Under the posterior scalp skin, the occipitofrontalis (epicranius) is identified by its occipital belly arising from the lateral two thirds of the superior nuchal line and adjacent mastoid region, then inserting into the galea aponeurotica. The galea spans anteriorly over the calvaria toward the frontal belly, while the occipital belly lies superficial to the occipital bone and deep to the subcutaneous tissue of the nuchal scalp. Inferior to this cranial landmark, the posterior superficial musculature continues into the cervical and truncal regions, with trapezius overlying the upper back and latissimus dorsi sweeping inferolaterally toward the humerus. Clinically, the occipital belly matters because it tensions the galea and moves the scalp as a unit, a relationship that explains why lacerations that cross the galea gape and bleed more than superficial cuts confined to skin and dense connective tissue. The loose areolar plane deep to the galea is also where subgaleal hematoma can track widely across the scalp, and it is the plane dissected in coronal approaches and many brow-lift or scalp-flap procedures when surgeons need broad exposure while protecting the supraorbital and occipital neurovascular bundles. A small muscle. A big surgical plane. Use this posterior under-skin view to teach scalp layers in gross anatomy, to illustrate facial plastic and reconstructive surgery approaches to the galea, or to support clinical education on scalp wounds, subgaleal hemorrhage, and tension-type headache discussions that reference the epicranius complex. It also works well as a comparative plate in posterior superficial muscle surveys when you want a clear cranial landmark anchored to the back and shoulder girdle.
