Description

Positioned on the posterior pelvic wall, the piriformis originates from the anterior (pelvic) surface of the sacrum and courses laterally through the greater sciatic foramen to insert on the superior border of the greater trochanter of the femur. A posterior view typically frames the muscle deep to gluteus maximus, with the gluteus medius and minimus lying superior and lateral, and the short external rotators (obturator internus, superior and inferior gemelli, quadratus femoris) stacked inferiorly. Medially, the sacrum and posterior sacroiliac region define the muscle’s proximal attachment, while laterally the greater trochanter provides a clear distal landmark. Orientation is the point. Clinically, this view helps you track the relationship between piriformis and the sciatic nerve as it exits the pelvis, a setup for deep gluteal pain syndromes when the nerve passes inferior to the muscle or, less commonly, through it. The same corridor matters during posterior approaches to the hip and during landmarking for safe intramuscular injection, since the superior lateral quadrant avoids the typical course of the sciatic nerve and inferior gluteal neurovascular bundle. Piriformis also provides a reliable reference when teaching hip external rotation in extension and abduction in flexion, because its line of pull shifts with femoral position.
