Description
Positioned high in the posterior hip of an adult male, the piriformis muscle appears as a pear shaped band running from the anterior surface of the sacrum to the superior border of the greater trochanter of the femur. Surrounding it in the gluteal region are the gluteus maximus superficially and portions of gluteus medius more superiorly and laterally, with the proximal hamstrings descending inferiorly along the posterior thigh. Bony landmarks include the iliac blade, the hip joint with the femoral head seated in the acetabulum, and the prominent greater trochanter lateral to the femoral neck. White tendinous insertions and fascial planes frame the muscular contours. Piriformis anatomy matters because it marks the transition between the pelvis and the deep gluteal space where neurovascular structures pass toward the lower limb. In most individuals, the sciatic nerve exits the greater sciatic foramen inferior to piriformis, but anatomic variants can place divisions of the nerve through or above the muscle, a setup associated with deep gluteal pain syndromes and procedure related nerve irritation. This relationship also guides the safe corridor for posterior hip injections and helps explain why tenderness near the greater sciatic notch can refer pain down the posterior thigh.
