Description
Sphenoid body (corpus ossis sphenoidalis) fills the frame in posterior (rear) view, emphasizing the rough, quadrilateral surface that meets the basilar part of the occipital bone at the spheno-occipital junction to form the clivus. Superiorly, this posterior face approaches the dorsum sellae and posterior clinoid processes, while inferiorly it transitions toward the sphenoidal rostrum and the midline crest that continues into the nasopharyngeal surface of the cranial base. Lateral margins sit medial to the petrous temporal bones in situ, framing the central basilar slope where the brainstem rests. A fixed posterior perspective makes the midline geometry obvious. The spheno-occipital region matters because it is both a key growth site and a surgical corridor. The spheno-occipital synchondrosis remains cartilaginous into adolescence before fusing, a point that affects pediatric skull base anatomy and cephalometric interpretation, and its mature bony interface contributes to the clival slope that underlies the pons and supports the basilar artery along the ventral brainstem. Clival chordomas and chondrosarcomas arise near this midline complex; surgeons planning an endoscopic endonasal approach to the clivus or an extended transsphenoidal route benefit from a clear understanding of where the sphenoid body ends and the occipital basilar part begins.
